This form is to register one student participant for our Confirmation Classroom Program during Fall 2026 - Spring 2027. If you are registering multiple students, please complete a separate form for each student.

The fee for this program is $200. Scholarships & payment plans are available upon request. If you plan on using a scholarship or payment plan, do NOT complete this form & please contact Sarah for more info.
