I give permission for myself/my child named above to participate in all meetings and related activites. I know of no reason I/my child should not be allowed to work with minors at Kenilworth Union Church. I understand there are inherent risks involved in all activities, including those inherent to being in contact with others during the COVID-19 pandemic, and I release the Church and all of its agents from any claim, whatsoever, arising out of activities associated with the CORE team.
